Plugin Icons not shown in toolbar

I've installed a number of plugins (New Item, File Tools, View Mode etc.). I do not seem to be able to get the icon's to show in the toolbar, even though I've selected them (moved them to the right) in the Customize dialog. I've seen the suggestion elsewhere that adding plugin icons only works when there is only one plugin installed. This kind of defeats the idea of using plugins. Is there something I'm missing?
