Under options, the only available options are Extra View (left), QT Command Bar (vertical), Extra View (bottom) and QTTabBar - Bottom. I've tried to restore defaults also uninstall and reinstall. Windows 10. Latest version of QTTabBar.

Lai, Thanks so much for your help. The install of classic shell was an improvement. Now both QTTabBar and QT Command Bar are available when I right click the classic explorer bar. However these options are grayed out and I'm not able to select them.
Under options/window Can you unCheck "Hide Menu bar".. This should get your menu bar back.
Then from the main menu drop down, go to view and tick the command bar you need.

Erixx Haxx, Thank you also for your help. There was no change after unchecking Hide Menu Bar. I'm starting to think that I've installed something that is interfering with it. I hate to do it, but a fresh OS install may be in my future.

You are not required to install Classic Shell, though you could keep it if you wish to. It is another great add-on to bring back some classics functions of Windows Start Button.
For your problem, I copied and rewrote the steps here:
1. Run Internet Explorer
2. Go to Tools -> Manage Add-ons
3. Find "QTTabBar" and make sure it is Enabled
The system may offer to enable all QTTabBar related add-ons, e.g. QT Command Bar 2, QT Base Toolbar, Extra View (left)… etc. Click Enable
4. Close Manage Add-ons
5. While still within Internet Explorer, go to Tools -> Internet Options
6. Go to the Advanced tab
7. Find "Enable third-party browser extensions" and ensure it is checked
8. Click Apply (if it is not grayed out), click OK
9. Exit Internet Explorer
10. Restart Windows Explorer, the options should be available now

You are the Man/Woman! I'm not sure if I removed IE or not but I could only find Microsoft Edge. I use Pale Moon/Firefox as my default browser so I was at a standstill until I remembered internet options can be found in the control panel. Manage Add-ons was found under the Programs tab. After enabling all QTTabBar Add-ons everything is right in my world. Thanks a million.
I think the options you need are under "Toolbars" and "Extra views" These sections deal with the menu bar placement
I thought it was broken, I was about to unistall.
Indeed this is a great piece of software for the missing functionalities of Explorer.
